FT KNOX SUPPORTIVE HOUSING INC. will not admit a resident unless his/her care and training needs can be met by the facility. That conclusion is based on a comprehensive review of the resident and their needs. It will include physical, emotional, social and cognitive factors all medical records. The administrator will determine if the admission is the best available plan for that resident.

A complete referral packet must be submitted prior to pre-placed interview to determine if the facility can serve the needs of the individual. A complete referral packet should include: Annual Review, Individual Program Plan, Psychological Evaluation, Medical profile (to include PPD) and previous program reports: i.e. IEP, IPP, and ISP both current and ongoing. The facility administrator will review all referral packets. Consultants will be utilized on an as needed basis. When determining appropriateness of a potential placement the Administrator, RN and or the Primary Physician shall schedule a pre-admission interview during which time all important information shall be gained.

All residents must be appropriate for FT KNOX SUPPORTIVE HOUSING INC. services. On an annual basis, the resident’s service coordinator shall make updates to reflect the current functioning level of each person. Listed below are the domains and the level of functioning that determine eligibility. In determining eligibility for admission, the facility administrator and RN consultant will complete the following:

  • Personal interview with the resident
  • RN will address ongoing monitoring of the resident’s medical status to ensure the continued appropriateness of the resident’s medical condition.
  • Personal interview with the resident’s parents/advocate if conserved, or if consent is given by the resident
  • Review of medical, dental, psychological, social and educational data
  • Appropriate consultation and coordination with Regional Center staff

The facility administrator and or RN will document the above and record all data in the resident’s records accordingly. Confidentiality of the resident’s records shall always be respected and maintained. If the resident appears appropriate based on the referral packet and intake interview, a pre-placement visit will be scheduled through the resident or liaison. If all parties agree after the pre-placement visit is completed, an admission date will be scheduled. Upon admission, clients or authorized representative shall sign the admission agreement, consents and rights forms.
