To Participate in Our Recuperative Care Program
- Homeless, at risk of being homeless, or in a place NOT FIT for Human Habitation.
- Independently able to perform all Activities of Daily Living (ADLs).
- Independently able use assistive device safely, properly, understands correct use, and without any assistance (e.g. wheelchair, cane, walker, etc.).
- Must be able to ambulate a distance of at least 100 ft. prior to Hospital / facility discharge, with or without assistive device.
- Able to self-administer medications (with staff oversight).
- Not be incontinent of bladder/bowel.
- Medically and psychiatrically stable at discharge. Alert and oriented to Name, Place, Date, and situation.

Exclusion Criteria
- Unable/unwilling to independently complete ADLs, personal care, and/or medication administration
- Incontinent of bladder and/or bowel and unable to self-care with adult briefs or other incontinence supplies
- Active Tuberculosis/C-DIFF/MRSA of sputum (possibly of wound) or other communicable/contagious condition(s)
- Medically and/or psychiatrically unstable
- Cognitively impaired (needs constant supervision and monitoring or re-direction and verbal cues for basic functions / ADLs)
- Recent combative, aggressive, or threatening behavior towards staff or other individuals
- Not able to live independently in housing and/or needs licensed care, such as needs skilled nursing, 24/7 care and supervision, medication administration, Board & Care
- Actively detoxing or withdrawing
